Acetyl-L-Carnitine (ALCAR) is a modified version of the amino acid L-Carnitine. The extra acetyl group lets it cross the blood-brain barrier, which plain L-Carnitine doesn't do well. That's why ALCAR is typically used for cognitive and neurological applications while plain L-Carnitine is more associated with energy metabolism and exercise. Evidence is moderate for a few specific uses. Studies at 1.5-2.5g/day show benefits for cognitive function in older adults with mild cognitive impairment or early dementia. There's also evidence for diabetic neuropathy (nerve pain from diabetes) and some trials showing antidepressant-level effects on mood in specific populations. Evidence is weaker for younger, cognitively healthy adults — if you're in that group, don't expect dramatic effects. ALCAR is sometimes labeled as "N-Acetyl-L-Carnitine" — same thing. Products combining it with plain L-Carnitine or Propionyl-L-Carnitine aren't included on TrueServing, since those blend multiple forms at different ratios.
