Alpha-lipoic acid (ALA) is a compound your body produces naturally in small amounts. It acts as an antioxidant in both water and fat, which is unusual — most antioxidants work in one or the other. Most ALA supplements are a 50/50 blend of two forms: the natural R-form and a synthetic S-form. This blend is what's been used in most clinical studies, including major trials on nerve health and blood sugar support. ALA is commonly taken for blood sugar management, nerve health, and general antioxidant support. Typical doses range from 300–600mg per day. It's available mainly as capsules. Some products add benfotiamine or biotin — those blends are excluded from our rankings to keep the comparison focused on ALA alone. Look for the alpha-lipoic acid amount and whether the label specifies the form. Labels reviewed for this update included plain Alpha Lipoic Acid at 600 mg, powder at 600 mg, an ALA blend of R- and S-alpha lipoic acid with no ratio disclosed, and add-on formulas with grape seed extract or BioPerine.
Monthly cost is driven by dose, bottle count, and form. Plain 600 mg capsules and bulk powder are usually easier to compare; liquids, low-dose nerve formulas, gummies, or products with many add-ons can become very expensive at a 600 mg/day comparison dose.
A label that just says Alpha Lipoic Acid usually does not tell you whether it is R-ALA, S-ALA, or a racemic blend. Do not treat R-ALA positioning, antioxidant language, nerve-health blends, or absorption add-ons as more alpha-lipoic acid unless the amount is listed clearly.
