What to look for

Phytosome mg and plain berberine mg are not the same thing. Berberine phytosome is a phospholipid-bound complex — the milligrams on the label include both the berberine and the lecithin carrier that improves absorption. This means 550 mg of phytosome is not equivalent to 550 mg of plain Berberine HCl. Clinical trials used 550 mg of phytosome twice daily (1,100 mg/day) and showed metabolic benefits comparable to much higher doses of standard berberine, so phytosome weight is the right unit for comparing these products. Berbevis is the form the research used. Almost every published berberine phytosome trial used Berbevis, the patented phytosome made by Indena. Other manufacturers sell "berberine phospholipid complex" or unbranded "berberine phytosome" products, and the phospholipid ratio, particle size, and absorption profile of those generics may not match Berbevis. Look for "Berbevis" or "Indena" on the label if you want the exact formulation studied. Unbranded phytosomes are not necessarily worse — just unproven.

Watch combination products. Some products bundle berberine phytosome with Alpha-Lipoic Acid, chromium, cinnamon, or — confusingly — additional plain Berberine HCl. Berberine inhibits CYP3A4, CYP2D6, and CYP2C9 liver enzymes, which means it can interact with a long list of prescription drugs — including metformin, statins, blood thinners, and immunosuppressants — so anyone on medication should check with a clinician before starting. Common side effects are gastrointestinal: constipation, cramping, or loose stools, though phytosome forms may cause less GI upset because less berberine sits in the gut. Berberine should not be used during pregnancy or breastfeeding.
